Twelve people were killed after a gunman forced his way into a California bar and opened fire on Wednesday night. The gunman threw smoke bombs when he entered, perhaps to cause confusion. Witnesses said some people broke windows in order to get out. A sheriff's deputy was among the dead. Twelve people were also injured. It happened in the city of Thousand Oaks which is "one of the safest cities in the country." The gunman was also found dead at the scene. He was shot, but it's unclear who shot him.
